Privacy Act Declaration: The information that you have supplied is necessary for the safe and effective operation of the after school programme, and has only been collected for that purpose. You may review information pertaining to your child's enrolment at any time.
In giving consent for my child/ren to attend FLOURISH After School Care, I agree to the following conditions:
FLOURISH After School Care provides themed days each week. We aim to give children broad opportunities and hope to include many areas such as craft, art, sports, games, music, dance, technology, science, reading & homework help, cooking & gardening.
FLOURISH runs from 2:45 –5:30pm Monday to Friday, during term time. It does not run on public or statutory holidays.
¨ Parents/caregivers are required to sign their child out of the program's daily on the attendance register provided.
¨ Staff will not release a child to a person who is not identified on the child’s enrolment form until the parent/caregiver has been contacted.
¨ Written permission must be provided for a child to leave the program unaccompanied.
¨ If a child is not collected at the end of the program a staff member will remain with the child & contact the parents/caregiver on the numbers provided. A late fee will be charged.
¨ Payments can be made via internet banking to: BNZ 02-0336-0040268-00 Reference -Last name of child
¨ If your child is enrolled on any given day and they are either sick or unable to come, please contact us before 2:00pm and let us know otherwise a “no show” fee of $5 will be charged to your account.
¨ Receipts and invoices will only be sent out upon request.
¨ After School Care will be discontinued after two weeks of non-payment.
¨ If your child is suspended from the programme, no refund will be given.
¨ If you cancel or if your child is sick/absent, we will not be able to issue a refund or swap days.
- Two weeks’ notice is required to un-enrol/cancel your child's care. Full payment is still required during those two weeks.
- Change of days may also require 2 weeks’ notice & is done only in consultation & at the discretion of the manager.
¨ WINZ subsidies:
It is suggested that you start this process before enrolling your child. Full payment of after school care is required until the subsidy has been approved.
WE WILL: ¨ Send a monthly calendar & newsletter home at least every term.
¨ Keep you informed of program changes & financial requirements.
YOU NEED TO:
¨ Notify us of your child’s absence as soon as possible, preferably before 2.30 pm to avoid unnecessary concernand time in tracking your child’s whereabouts.
¨ Inform us of any changes to approved people to pick up your children.
¨ Ensure that your contact details are up to date at all times or any changed conditions/information
Should you have any queries, concerns or feedback, please speak to FLOURISH Staff.
Feedback forms are available for any positive, negative or constructive feedback you would like to give us.
Medication, Accidents & Emergencies & Sickness
¨ Parents/caregivers must complete a medical form if any serious medical condition is noted on enrolment form. This will include; written permission for staff to administer medication.
¨ All medication is to be held by supervisor or support staff.
¨ All accidents are reported in the program's Accident Book.
¨ If your child is ill, keep her/him home for at least 48 hours. If a child becomes ill during the day, parents will be called and asked to collect their child.
Policies & Procedures ¨ A copy of our policies and procedures is available at the centre.
¨ Do not allow your child to bring valuables to the programme. FLOURISH accepts no responsibility for damaged or lost items.
¨ To ensure the safety of all children & staff, we run a “3 strikes and your out” behaviour system. A strike is a recorded incident and on receiving 3 strikes we have no choice but to suspend the child. Attendance back at FLOURISH will then be after consultation with the parents & staff.
